Request Certificate of Insurance

A certificate of insurance (COI) is a document from an insurer to show you have liability insurance for an event. They’re also known as certificates of liability insurance or proof of insurance.

It’s common to need a COI when planning an event. Venues often require hosts to have insurance and may contractually require the host to name the venue as an Additional Insured. A COI provided to the venue contains the information they need to confirm that you are properly insured.

Under CUPA-HR’s insurance plan, you have the option to request proof of liability for specific chapter events. To request a certificate of insurance, complete the Request for Certificate of Insurance form on this page. To ensure you receive the certificate in time, please email your request at least one week prior to your event.
