Download a direct deposit form.

Direct Deposit Information:

  • If you wish to change the current information on file, then fill out the direct deposit form completely, selecting Change Existing information on the form, and return it to Flores. It will take approximately three weeks to verify the information through the banking system before it will become active. If any reimbursements are owed during the verification period this will be issued as a live check.
  • If you wish to remove the direct deposit information on file, then fill out the general information section of the direct deposit form, select Cancel Direct Deposits, sign the bottom of the form and return it to us. It will take approximately two business days for the direct deposit information to be removed. Once the direct deposit information is removed then reimbursements will be issued as a check.

Flores Medical Care Reimbursement Tip: Combine Rx Claim Lines

  • Are the receipts for the same patient and from the same pharmacy? If so, did you know that it is not necessary that you complete a separate line item on the claim form for each Rx? When both the patient's & provider's name are the same, you can complete one line item on the claim form requesting the total amount of all of the receipts as long as they are all from the same plan year.

403(b) Retirement Plan

One-on-One Financial Counseling Sessions:

  • One-on-one financial counseling sessions are offered at UT to review your retirement portfolio, allocation, risk tolerance, and to answer any retirement questions you may have. You can also learn about other services TIAA-CREF offers such as life insurance, mutual funds, and IRA's.
  • The sessions are free and one hour in length. They are held at the UT Conference Center. You can park in the Locust street garage and parking is validated.
  • Click on the link below, then select Tennessee and scroll down until you see the sessions for University of Tennessee at Knoxville. Select a date, then scroll down to event information and click the radio button on the time you would like to meet. Fill out the personal information on the page and then click submit. You will receive a confirmation email, a reminder email, and a document to complete listing your personal financial information (savings, net worth, cash, retirement, etc). You will take the document to your meeting and they will use that information along with your retirement plan with TIAA-CREF to assist you in your retirement planning.
